Do you take my insurance?

Yes! We are in network with many insurance plans and payers including Medicare, InterCommunity Health Network (IHN), Samaritan Advantage, Samaritan Choice, Oregon Health Plan, and Blue Cross Blue Shield/Regence (BCBS), Pacific Source, Cigna, and VA. We are in the process of becoming an in network provider with several other insurance payers. We also provide services for cash payment for a flat fee.

What should I bring to my appointment?

For your physical therapy evaluation, we ask that you wear comfortable, modest clothing as you will likely be going through movement testing and potentially exercise. Athletic footwear is preferred but not required. Please wear what makes you feel comfortable.

Do I need a referral?

You do not need a referral to receive services at Lebanon Physical Therapy. Some insurances including Medicare will require a physician signature within 30 days of the initiation of treatment. Your private insurance may not require this. Lebanon Physical Therapy will take care of the approval process if you choose to participate in therapy prior to receiving a referral from your primary care provider.

What can I expect at my first appointment?

Physical therapy visits are 1:1 and will last around 40 minutes. Plan to answer questions and collaborate with your therapist to demonstrate your current level of function, and establish therapy goals. Movement is a fundamental piece of physical therapy, expect to move as much or little as you can tolerate. Together we can work through challenges and achieve new goals.

Do you have a cancellation policy?

If you are not able to make your scheduled appointment, please call the clinic at 541-401-6500 to let your therapist know. We ask that you provide 24 hours notice in order to make schedule adjustments.
